(1st page only. Most reports are 3-5 pages with explanations)

Project: XYZ Maryland
Lab Report: #001234/Analysis by David T. Hanson
Date: Dec 31, 2001
Engineer: N/A
Contractor: ABC Drilling

Sample Number N/A Bacterial Identification, Counts, & Information
Time N/A
Nutrient plate count 22 colonies per milliliter
Bacterial Identification Commamonas testosteroni
ATP count 122,000 million cells per milliliter
Sulfate Reducing Bacteria Positive
Microscopic analysis Minor bacterial growths, no iron oxidizing bacteria noted
Notes Iron 0.5 ppm, TDS 295 ppm
Coliform Bacteria Negative
Fecal Coliform Negative

Interpretation
Seven separate observations are used to identify bacterial problems, 1 & 2. Nutrient plate which includes identification and colony counts of aerobic bacteria, 3. ATP which is a separate count of all living organisms in water, 4. Microscopic Analysis which is a filtered sample that includes ID of any iron bacteria present along with any iron oxides from decaying organisms, and 5. Notes which allows some other points of chemical interest. 6. Coliform bacteria. and 7. Fecal coliform bacteria.
